Sealants have been available for almost 25 years, but many dentists do not take full advantage of them. They received approval from the American Dental Association (ADA) Council on Dental Materials in 1976. There is one main rule in dentistry. If you can keep it clean, you can keep it. Brushing and especially flossing go a long way in the prevention of cavities and gum disease. What happens if a tooth is not cleanable? I have to make it cleanable or the inevitable cavity and/or gum disease will ravage the tooth. There is a design flaw in teeth, grooves on the tops of back molars that do not allow for the tooth to be cleaned naturally or with a brush. Because of the likelihood of developing decay in the depressions and grooves of the premolars and molars, children and teenagers are obvious candidates for sealants. However, adults without decay or fillings in their molars can also benefit from sealants. Typically, children should get sealants on their permanent molars and premolars as soon as these teeth come in. In this way, the sealants can protect the teeth through the cavity-prone years of ages 6 to 14.

In order to bond with tooth structure, the tooth must be clean. Etching the tooth surface is required. The acid used to etch teeth is usually phosphoric acid. This can be used in varying strengths, but the most commonly used strength is 37 percent. The acid etch serves several purposes. It removes debris from the tooth surface, provides better wetting capability of the enamel for the resin and selects dissolution of the enamel surface. Enamel is normally porous but etching does remove some of the calcium salts to increase microspaces. These allow “tags” of resin to penetrate the enamel for better bonding. Most often after eating a few meals, the taste and small bite discrepancies will generally disappear. The other consideration a dentist mentions is that if the sealant is not done extremely well, they can actually lead to very serious cavities under the seal. Chewing ice and sticky foods may decrease the life of sealant. Every patient who has had sealants placed should be examined when they return for hygiene visits. If sealants are missing, they should be reevaluated to determine if replacement is necessary. Sealants can last up to 10 years. But they need to be checked at regular dental check-ups to make sure they are not chipped or worn away. Mercury is a metal that occurs naturally in the environment. Mercury can exist as a liquid, as in many thermometers. When heated, it becomes a gas. It also can be combined with many other materials. Everyone is exposed to mercury through air, drinking water, soil and food. Concerns have been raised, for instance, about the amount of mercury building up in fish as a result of pollution. Mercury enters the air from industries that burn mercury-containing fuels. Mercury from all sources can accumulate in body organs. As with most substances, the degree of harm caused by mercury in the body is related to the amount. Very low levels don’t cause any ill effects. At higher levels for instance, when workers are exposed to mercury through their jobs mercury can cause several symptoms. These include anxiety, irritability, memory loss, headaches and fatigue. The controversy over amalgam centers on how much mercury is released from fillings and absorbed into the body. In the past, amalgam fillings were thought to be inert. This would mean that no mercury was released once the filling was complete. In recent years, sophisticated tests have changed this view.
